If you wish to send us a referral please complete the form below:


Referral Form

  • If you are currently in receipt of a Direct Payment and wish to use our services please contact your allocated social worker and pass on our details so we can arrange this for you.

  • If your Direct Payment is currently being managed by another provider and you wish to swap to ILBP please contact your social worker and pass on our details so we can arrange for this to be transferred.

  • If you are currently in receipt of a Personal Health Budget and wish to use our services please contact your Nurse Assessor and pass on our details so we can arrange this for you

  • If your Personal Health Budget is currently being managed by another provider and you wish to swap to ILBP please contact your nurse assessor and pass on our details so we can arrange for this to be transferred.
